In an effort to provide you with outstanding service, and to
better orient you within our facilities, we have developed
the following patient guidelines. Please read them. If you
have any questions, please ask. Our goal is to make your visits
as productive and pleasant as possible.

Getting Started
|
|
Patients may come to Healthwell for services through a physician or
as a self-referral. Call Healthwell to schedule your appointment.
Bring your physician's prescription for treatment if you have one,
and your insurance information if your want your insurance company
to be billed.
On your first visit to Healthwell, plan to arrive 15 minutes early to
park, fill out necessary forms, and change. Wear comfortable clothing.
Patients with knee injuries should bring a pair of shorts.
Bring a folder to keep schedules and home exercises organized,
especially in cases of litigation.

Appointment Scheduling
|
|
We strongly recommend that you make your appointments well in
advance to ensure the optimum time for you, and to avoid delays
and breaks in treatment. Please schedule with the front desk
between 8:30 and 5:30, Monday through Friday. We are closed for
lunch from 1:00 to 2:00. To leave a message after hours, please
dial extension 201.

Reminders
|
|
As a courtesy, our staff will try to confirm your appointment the
day before. Please be aware that you are still responsible for giving
24-hour notice should you need to reschedule or cancel an appointment,
regardless of whether or not you've received a call.

|
Cancellations
|
|
We require a minimum of 24-hour notice should you need to cancel
or reschedule an appointment. Missed appointments and appointments cancelled
less than 24 hours beforehand will be billed to you.

|
Payments
|
|
In an effort to provide cost-effective services to you, all private
pay patients are required to make their payment at the time
of service. Visa and MasterCard are accepted.
|
|
Office Records & Chart Notes
|
|
All patient records are kept strictly confidential. We must have a
signed authorization on file to process your insurance claims.
To provide a copy of your file, we must also have a signed release.
|

Insurance Coverage |
|
Many of our patients are able to use medical insurance to help
defray the cost of their treatment. However, the vast majority of
insurance providers do not cover all treatment in full. Patients
are responsible for any deductibles, co-insurance and/or copay
amounts. We cannot guarantee eligibility, extent of coverage or
any payment, but as a courtesy to our patients, we will attempt
to intercede on your behalf. Patients are, however, ultimately
responsible for any outstanding balance.
The PPO insurance companies listed below have paid for some
portion of services rendered at Healthwell. Please contact your
insurance provider directly for the specific details of your plan.
If requested, Healthwell may check your benefits prior to your first
visit. However, benefits given to us on your behalf are not a guarantee
of payment by your insurance company, as providers decide at the time
of each claim how much they will allow.

In addition, Healthwell accepts Medicare and all automobile insurances. We are not providers for
any HMO insurances, or for most Brown and Toland plans.
It is important to note that most often, unless treatment
is rendered for a Workers' Compensation injury, some form
of co-payment or co-insurance is required from the patient.
Patients are asked to render their payments in a timely manner.
