Reminders

We make every effort to remind you of your appointment at HEALTHWELL San Francisco Physical Therapy Offices. As a courtesy, our staff will try to confirm your appointment the day before. Please be aware that you are still responsible for giving 24-hour notice should you need to reschedule or cancel an appointment, regardless of whether or not you've received a reminder call.

Cancellations

We require a minimum of 24-hour notice should you need to cancel or reschedule an appointment. Please call us at 415.921.1211 to cancel or reschedule. We cannot consider an email message cancellation notice. You will be charged for missed appointments and appointments cancelled less than 24 hours beforehand.

Payments

In an effort to provide cost-effective services to you, all private pay patients and patients who have a deductable over $1000 are required to make their payments at the time of service. Visa and MasterCard are accepted. Copies of your bill are available from the billing office.

Office Records & Chart Notes

All patient records are kept strictly confidential. We must have a signed authorization on file to process your insurance claims. To provide a copy of your file to another health care provider, an attorney, or any other interested party, we must have a signed release.
